The services listed have been established without regard to your medical insurance status. Laboratory and/or diagnostic test fees are not included in the rates.



Consultation $250 - $400

May include, but is not limited to: Bio-identical hormones, cholesterol (lipid) management, depression/anxiety, dietary/nutritional issues (IBS, gluten sensitivity), herbal/dietary supplement use, meditation/Mind-Body-Spirit techniques, Menopause/Perimenopause, metabolic conditions (chronic fatigue, fibromyalgia and hypothyroidism), panic attacks and weight loss/management counseling.

Laboratory costs not included in price.

Urgent Care Visit / Office Visit $150 - $300

One complicated issue or for 2-3 uncomplicated issues.

Annual Well Woman only (all ages) $320

Complete breast and pelvic exam (Price includes cost of Pap smear lab). (Combined Well Woman + Physical Exam $450)

Additional laboratory/diagnostic tests and/or cost of cultures or other tests may be ordered and are not included in the price.*

Adult Physical Exam (40 yrs and up) $375

Complete medical history, physical examination, pulmonary function tests and body fat composition.

Exam includes EKG and hemoccult cards for annual stool evaluation. Laboratory costs not included in price.*

Young Adult Physical Exam (13-29 yrs) $300

Complete medical history, physical examination, pulmonary function tests and body fat composition. Additional laboratory/diagnostic tests may be ordered if needed.

Acupuncture Initial $120, Per Session $90

Full treatments usually consist of 2-3 sessions.

Disclaimer

While the rates of service are updated frequently, they may change over time and it is important to check prior to the start of each visit for the most current rates of services. Payment-in-Full is expected at the time of service and exceptions for payment CANNOT be made.

Communication

Phone calls / e-mails

Patients are encouraged to call the office for any issues, however, under certain circumstances patients seeking medical advice/attention will be charged for the necessary services rendered. Phone calls/emails are not to be considered a substitution for office visits. The need for phone call/e-mail visits, as opposed to office visits, will be determined at Dr. Counts or Traci’s discretion. E-mails requiring medical advice are billed at the same rate as phone calls.

Phone calls and emails for acute problems will begin at $125, depending on the time spent. (Charges may increase depending on time/issue.)

Follow-up calls

We understand life is busy and for your benefit, if you were recently seen in the office and labs/imaging were ordered, for your convenience and to expedite your medical care, Dr. Counts will offer a phone follow-up for most low complexity cases for $125 for up to 15 minutes.

We value our ability to care for our patients and these new charges provide our office compensation for our time and effort.

Payments

Dr. Counts does not accept any insurance and is considered an out-of-network provider. We are a fee-for-service practice which means payment is due at the time of service. If you have insurance and wish to file the office visit, we will gladly generate an invoice for you to submit for possible reimbursement. (Please call the number on the back of your insurance card to inquire if your plan covers out-of-network providers).

The services listed have been established without regard to your medical insurance status. Laboratory and/or diagnostic test fees are not included in the below rates. If you have medical insurance, you will be able to utilize your insurance for your laboratory tests. However, if you do not have medical insurance and laboratory studies are needed, we can offer most studies at discounted rates.

Dr. Counts accepts cash, VISA, MASTERCARD and Discover. You may also make payment using your HSA or FSA debit card, as long as it has a VISA or MasterCard logo.

Please note: We currently see patients between the ages of 13-70. Please call with any questions.

Medicare Patients

Dr. Counts has also opted out of Medicare, however, he will see Medicare patients via private contract. By signing a Medicare private contract with Dr. Counts, you are agreeing not to submit claims for the office visit with Dr. Counts as you will not be reimbursed for the visit. Payment is due at the time of service. For tests/procedures ordered outside of Dr. Counts’ office, you will be able to use your Medicare benefits.

The Medicare Program

Q: What is a "private contract," and how does it work?

A: A private contract is an agreement between you and a doctor who has decided not to give services through the Medicare program. If you sign a private contract with your doctor:

  • Medicare won't pay any amount for the services you get from this doctor.
  • You will have to pay whatever this doctor or provider charges you for the services you get. Medicare's limiting charge will not apply.
  • Medicare + Choice plans will not pay for these services.
  • No claim should be submitted, and Medicare will not pay if one is submitted.
  • Your Medigap policy, if you have one, will not pay anything for this service. Call your Medigap insurance company before you get the service is you have any questions.
  • Many other insurance plans will not pay for the service either.
  • Your doctor must tell you whether Medicare would pay for the service if you get it from another doctor who participates in Medicare.
  • Your doctor must tell you if he or she has been excluded from the Medicare program.

The private contract only applies to the services you get from the doctor who asked you to sign it. You cannot be asked to sign a private contract in an emergency or urgent health situation.

You may want to talk with someone in your State Health Insurance Assistance Program before signing a private contract.
