While the rates of service are updated frequently, they may change over time and it is important to check prior to the start of each visit for the most current rates of services. Payment-in-Full is expected at the time of service and exceptions for payment CANNOT be made.
We value our ability to care for our patients and these new charges provide our office compensation for our time and effort.
Dr. Counts does not accept any insurance and is considered an out-of-network provider. We are a fee-for-service practice which means payment is due at the time of service. If you have insurance and wish to file the office visit, we will gladly generate an invoice for you to submit for possible reimbursement. (Please call the number on the back of your insurance card to inquire if your plan covers out-of-network providers).

If you have medical insurance, you will be able to utilize your insurance for your laboratory tests. However, if you do not have medical insurance and laboratory studies are needed, we can offer most studies at discounted rates.
Dr. Counts accepts cash, VISA, MASTERCARD and Discover. You may also make payment using your HSA or FSA debit card, as long as it has a VISA or MasterCard logo.
Please note: We currently see patients between the ages of 13-70. Please call with any questions.
Dr. Counts has also opted out of Medicare, however, he will see Medicare patients via private contract. By signing a Medicare private contract with Dr. Counts, you are agreeing not to submit claims for the office visit with Dr. Counts as you will not be reimbursed for the visit. Payment is due at the time of service. For tests/procedures ordered outside of Dr. Counts’ office, you will be able to use your Medicare benefits.
Q: What is a "private contract," and how does it work?
A: A private contract is an agreement between you and a doctor who has decided not to give services through the Medicare program. If you sign a private contract with your doctor:
The private contract only applies to the services you get from the doctor who asked you to sign it. You cannot be asked to sign a private contract in an emergency or urgent health situation.
You may want to talk with someone in your State Health Insurance Assistance Program before signing a private contract.
